In filling out this application form, I understand that TALIS Construction Corporation may wish to investigate any of the facts or statements submitted by me. I hereby grant it permission to check any of this information except where my written statement upon this form specifically requests that no investigation be made. (This includes reference checks with present employers.)
I also understand that any falsification, deliberate omission, or misrepresentation of facts upon this application will be considered just cause for dismissal at the discretion of the company should I become an employee of TALIS Construction Corporation.
I understand that TALIS Construction Corporation does not intend to alter, by the words or actions of the company or its employees, the traditional rule that either the company or its employees may terminate the employment relationship at any time. I also understand TALIS Construction Corporation does not intend to create an implied contract between the company and its employees through the development and dissemination of company policies, procedures, handbooks, or other literature.
By signing this statement I understand that TALIS Construction Corporation may request an investigative consumer report from a consumer-reporting agency to verify any of this information including, but not limited to, criminal history and motor vehicle driving records. I further understand that I have the right to request In writing to the personnel department within a reasonable period of time from the date of this application for disclosure of the name of the investigating agency and a complete and accurate disclosure of the nature and scope of the investigation requested.
In accordance with TALIS Construction Corporation's policy to maintain a drug-free workplace, I understand that I may be required to submit to a test for drugs. Employment with TALIS Construction Corporation is contingent upon a negative urinalysis drug screen.
TALIS Construction Corporation requires steel toe safety shoes to be worn at all times. TALIS may issue tools or safety equipment for the use of employees. I agree that If I am employed by TALIS I will surrender any such tools and equipment issued to my supervisor upon termination of employment. I agree that if I fail to surrender such tools and equipment, TALIS Construction Corporation after duly notifying me and providing me an opportunity to address the failure to surrender such tools and equipment, may deduct the fair value of such tools and equipment from my final payroll check. Prior to receiving a final paycheck a mandatory exit interview between the employee and the employer will take place at which time the employee is required to sign the termination notice.
